An unnamed woman delivered him and his twin brother Jacob but killed their mother and raised them herself. Jacob was swaddled in light cloth while The Man in Black was wrapped in dark. At age thirteen, the twins discovered other survivors of Claudia's ship. After Claudia's ghost revealed the truth about their adoptive mother to The Man in Black, he left to join the crew. When his adoptive mother learned this, she massacred the camp and destroyed a well where The Man in Black planned to install a wheel to use the island's electromagnetic properties to leave. Enraged, The Man in Black killed her with a dagger. Jacob retaliated by throwing him into a stream leading into a tunnel of light inside a cave filled with light known as 'The Heart of the Island'. The Smokescreen Deception

In 1867, The Man in Black and Jacob conversed on the beach as the Black Rock ship approached. He expressed disapproval of Jacob bringing people to the island and stated he wished to kill him, but their adoptive mother had prevented him from hurting Jacob, so he looked for a 'loophole'. After the Black Rock crashed, The Man in Black killed everyone except Richard Alpert. He then appeared as Isabella (Richard's dead wife) to trick Richard into thinking they were in hell and that Jacob had taken her. Later, in his own form, he freed Richard and gave him a knife to kill Jacob, whom he called 'the devil', though Jacob convinced Richard otherwise.
