Jack was born into a successful family and attended Columbia University, graduating from medical school a year earlier than his classmates. His backstory involves trauma regarding his father, Christian Shephard; after finding Christian's body in Australia, Jack brought him home on Oceanic 815. On the island, he initially acted as a man of science, opposing John Locke's faith-based views and repressing fear to remain strong for others. He developed relationships with Kate Austen, Hurley Reyes (as a right-hand man), and Sayid Jarrah (with mutual respect). After being rescued, Jack suffered from severe depression, alcoholism, and drug abuse, blaming himself for leaving other survivors behind. This led to delusions about his father and a suicide attempt. Visited by Locke to return to the island, Jack changed his outlook, adopting faith in destiny with the belief that he was "supposed" to go back. He ultimately sacrificed himself to prevent the island's destruction, engaging The Man in Black (who appeared as Locke) in a climactic battle.

Jack boarded Oceanic Flight 815 on September 22, 2004, with his father's casket in the cargo hold. After the crash, he instructed survivors to help the injured and used his medical background to treat the wounded. Exhausted and sleep-deprived, he began chasing hallucinations of his father until discovering an empty coffin in the jungle. He returned to the beach camp to deliver a speech about living together and dying alone, establishing caves as a base. As the de facto leader for the first four seasons, he managed crises while repressing his own anxiety. After escaping, he moved in with Kate Austen and raised Claire's son, Aaron, but fell into depression. Following Locke's persuasion to return, Jack embraced fate, accepted his role as protector of the island, and died after defeating The Man in Black.
